- Print off the Assignment of Benefits Form, sign it and give it to your doctor.
- Fill out the customer inquiry form on our Contact Us page and we’ll contact you for the information we need to process your order. We’ll need your:
- Wound assessment
- Progress notes
- Demographics
- Proof of debridement
- Sales order
- Detailed treatment plan
